We will need a referral from a UK registered doctor, with the patient's full contact details, diagnosis, scan required, and information needed. Your doctor can provide this using the form, or alternatively please call us if he or she would like a referral pad. If you are insured, please contact your insurance company to obtain an authorisation code.
